About Health Everywhere

Health Everywhere is a Washington, DC based collaboration of remote patient monitoring (RPM) policy and industry stakeholders dedicated to providing Medicare beneficiaries’ broad access to RPM. Our goal is to contribute to better patient outcomes and lower costs for the healthcare system through the use of home-based RPM services that supplement care management by providing a range of clinical and supporting information. Our efforts are focused on enhancing federal healthcare policies in order to provide Medicare beneficiaries access to RPM through Medicare coverage and reimbursement.

About Remote Patient Monitoring

RPM is a coordinated system that uses one or more home-based monitoring devices that automatically transmit vital sign data and responses to assessment questions.  The data are collected wirelessly or through a telecommunications connection to a server that complies with the HIPAA privacy and security rules. RPM includes the review and interpretation of vital sign data by a health care professional as part of an established plan of care for a patient. Users of RPM are typically seniors and people with disabilities, especially those with chronic conditions such as congestive heart failure (CHF), diabetes and/or chronic obstructive pulmonary disorder (COPD).  Using RPM to manage chronic conditions improves quality of life, engages patients in their care, communicates vital information about their health to healthcare professionals, and avoids  hospitalizations and unnecessary provider visits.

For example, our devices remotely monitor weight in patients with CHF. By detecting slight changes in weight and asking a series of related clinical questions, RPM helps avoid exacerbations of illness that could lead to emergency room visits, hospital admissions and readmissions. Similar examples exist for diabetes and COPD. RPM can improve care for patients, manage population health, and lower costs Medicare program costs.
